I certify that I am the owner (or authorized agent for the owner) of pet listed above and am 18 years of age or older. I further agree that I/we will be responsible for all the decisions regarding this pet's health and welfare, legally, ethically and financially.
Southwest Virginia Veterinary Services (SVVS) has business and medical staffing hours from 8:30 a.m. through 5:00 p.m Monday, Tuesday, Thursday, & Friday and from 8:30 a.m.-12:00 p.m. on Wednesday. The office is closed on Saturdays and Sundays and the following holidays: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Eve, Christmas Day and the Day after Christmas. We offer Sunday pick up from boarding between 4:30 and 5:00 pm, unless that Sunday is a holiday and we will not have pick up. Emergency services are offered for our clients by calling our office number and triaged by our anwering service (GuardianVet) each day until 8PM Monday through Thursday. We will not offer on-call services on any holiday or weekend (Friday to Sunday). There is an additional emergency fee for pets that need to be seen outside of normal operating hours.
This hospital DOES NOT have full-time, continuous medical staffing; however, critically ill patients are frequently monitored throughout the night by the emergency doctor. On Saturdays, Sundays and the holidays listed above, our staff is limited to kennel personnel and the emergency duty doctor.
By signing this form, I indicate that I am aware of the above listed staffing hours and am consenting and authorizing the doctors and staff of Southwest Virginia Veterinary Services to hospitalize my pet, to administer vaccinations, medications, tests, surgical procedures, anesthetics, or treatments as discussed and that are deemed necessary for the health, safety and/or well-being of my pet while he/she is under their care and supervision.
Southwest Virginia Veterinary Services Payment Policy
All fees for services and products are due when rendered. Payment for all other services will be made directly to Southwest Virginia Veterinary Services (hereafter referred to as SVVS).
If you have pet health insurance, filing the claim will be your responsibility and you will still need to pay SVVS when services are rendered.
We accept cash, personal checks (with photo ID), money orders, traveler’s checks, American Express, Discover, Mastercard, Visa, CareCredit and AllPet.
If you pay with a personal check, you agree that using the check as payment constitutes a contractual agreement. If your check is returned, it may be re-presented electronically or by paper draft and you will be responsible for any and all penalties, collections costs, and incidental damages allowed under Virginia law. IF returned, your check will automatically be presented to PayTek Solutions for collection and will not be returned to SVVS.
SVVS has a no show/late cancellation policy and will charge your account $25.00 if you do not cancel your appointment in a timely manner, if you do not show up for your appointment or if you cancel or reschedule an appointment 3 or more times.
I understand that payment is expected when my pet/s is discharged from Southwest Virginia Veterinary Services and that for Sunday pickup, pre-payment is required. Additionally, for the safety of our employees, and for the protection of your pet, SVVS will require clients who use our Sunday pickup services to show a photo ID to our kennel attendants when you arrive to pick up your pet.
A deposit will be required for new clients and/or patients and major medical/surgical cases of trauma/emergency care when your pet is hospitalized.
Services requiring hospitalization must be paid in full before my pet/s may be released unless other arrangements are made with the office manager.
In the event of non-payment, you will be responsible for any finance, collection or attorney fees.
There is a 24% per annum (2% per month) finance charge applied to any outstanding balance over 30 days.
Social Media and Website Consent/Release Form
I, new, do hereby give permission to SVVS and SVVS Mountain Wildlife Care Inc. to record, publish, and republish stories, photos, videos, and items, hereinafter referred to as “materials”, which I submit for SVVS and SVVS Mountain Wildlife Care Inc.’s website and social media.
I hereby release SVVS from all claims and demands arising from any use of said “material”, including, without limitation, all claims of invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights.
I acknowledge and agree that no sum whatsoever will be due to me because of the use and/or exploitation of the “materials” or any rights therein.
